AdventHealth seeks to hire a Care Navigator - Magic Parties who will embrace our mission to extend the healing ministry of Christ.

The Care Navigator - Magic Parties coordinates all services required to provide multidisciplinary, quality assured, appropriate case selection and cost-effective care to Magic Parties. Patient centered representative who works on behalf of AdventHealth's (FH) preferred and partner networks to enhance the overall patient experience for members of the Magic Parties (Orlando Magic, Solar Bears, and Lakeland G League). Responsible for the direct relationship with the Sr. VP of People, Director of Player Development (or their designee), and Chief Operating Officer of the Magic Parties for the purposes of seamless, timely access to, and step-by-step coordination within the FH network. Interacts with patients, families, physicians, and medical staff to facilitate a proactive and positive health and wellness experience. Establishes relationships with patients by providing emotional support through active listening, utilizing healthcare experience and providing quality customer service effectively throughout interactions. Makes initial contact with the organization/patient/family to establish a professional relationship and understanding of the diagnosis, treatment and follow-up regimen. Assesses the patient's status and works with physicians to determine appropriate plan for patient and assess appropriateness per regulatory and vendor guidelines. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
Knowledge of AdventHealth's network infrastructure and roles of the interdisciplinary staff and service personnel
Knowledge of the Preferred Provider Networks, including the Clinically Integrated Network (CIN) staff, services, and processes
Healthcare knowledge to provide effective and appropriate support to the Magic Parties members
Ability to recognize and safeguard confidential information, maintain HIPAA guidelines, and provide superior discretion as it relates to high profile consumers or members
Ability to deal with senior managers, company executives, community leaders, athletes and their families
Strong customer service skills, and ability to handle difficult patients and families related healthcare issues
Ability to build rapport and work in a supportive way with the patient, family and the healthcare team to troubleshoot and resolve issues
Ability to handle stressful situations, work on multiple projects, be flexible and adaptable to change
Planning and organizational skills, with emphasis on documenting appropriately
Flexibility in prioritization; ability to multitask; critical-thinking/problem-solving skills
Self-motivated with ability to work independently and in a group setting
Strong organizational skills
Ability to observe assess, guide and evaluate others
Ability to communicate timely and in an open and honest manner
Computer skills in Microsoft Office (Outlook, Word, Excel)
Ability to learn various scheduling and customer relationship management software
Bachelor's degree in Nursing or Athletic Training
10 years of experience in clinical healthcare working as a nurse or an athletic trainer
5 years of experience in service of operations management
Master's degree in Business, Healthcare Administration, Marketing, or related field (Preferred)
Licensure, Certification, or Registration Required:
Current valid State of Florida or multi state license as a Registered Nurse, OR National Athletic Training Association (NATA) certification

Acts as a liaison between the Magic Parties, patient, and the AdventHealth network and partner networks by regularly meeting with Magic Parties to develop a smooth process for access to care and communication.
Manages and develops processes across FH Network for timely execution of partner's needs for services, consultations, and ensures complete resolution and communication of inquiries or concerns brought forward.
Manages patient education and care: serves as an ongoing resource for patients and families during their diagnosis and treatment period; communicates information regarding patient status and needs; collaborates with other providers regarding how to best meet needs and sequence care; assesses patients' learning style and health literacy in order to provide education on the appropriate patient level.
Provides telephone and face-to-face consultation with patients to answer pertinent questions throughout their hospital experience; provides educational information to patient/family and reviews information with patient at discharge and/or within 48 hours post-discharge. Communicates with patient and family 24 hours after discharge to clarify any outstanding concerns to ensure patient understands follow-up.
Keeps Supervisor and Director informed and seeks assistance and direction from appropriate individuals when confronted with unfamiliar procedures and techniques before implementing them to ensure delivery of safe and effective patient care.
Communicates and collaborates with multidisciplinary team to ensure high-quality patient care. Acts as a liaison to physicians' offices regarding proper orders, medical records, consents and any other required documentation/criteria ensuring they are received at the time of scheduling a patient for a procedure.
Logs information (non-HIPAA information only) on Customer Relationship Management system.
Develops, plans and implements evidenced-based measures that increases the quality for the full spectrum of patient care and ensures compliance with all regulatory guidelines.
Navigates a diagnosed patient from the diagnostic phase through the treatment phase, ensuring appropriate/timely medical/supportive services.
Adheres to after-hour on call schedule as established for 24/7 or 365 access by Magic Parties.
