The Staff Pharm II works in the Specialty Hub to provide services to patients and assist patients in getting their medications. The pharmacist's responsibility is to monitor drug therapy to achieve desired outcomes and to avoid drug toxicity and the dosing of medication based on patient demographics, available treatment guidelines and response. The pharmacist works as an integral part of the patient care team and assists physicians and nurses in providing drug therapy.REQUIRED-Valid California Pharmacist License-PGY-1 Pharmacy Practice or Specialty residency or relevant BPS or CSP certification or minimum of 3 years of relevant clinical experience required-Clinical pharmacy training,including familiarity with medical terminology, lab data and optimal drug therapy-Thorough knowledge of pharmacy laws and regulations-Thorough understanding and competency with aseptic technique and safe handling of hazardous meds-Computer skills that facilitate Clinical patient data medication order entry and other basic clinical software programs-Demonstrated ability to effectively communicate with and educate patients about their medication therapy-Demonstrated ability to obtain needed information from a patient or patient's chart to create medication management plans-Written and oral communication skills to clearly and concisely provide information with exceptional interpersonal skills to healthcare providers-Demonstrated ability to triage-Familiar with Physical assessments and the use of physical assessment monitors such as DM machines, asthma spacers/inhalers, BP monitors, coagucheck machines, etc-Knowledge of current ambulatory care guidelines (i.e. NCEP, JNC, ADA).-Demonstrated ability to identify medication-related problems in patients, communicate them to a patient's physician and offer recommendations-Complete working knowledge of, and skill to apply, practices, principles and techniques of patient care related to area of expertise-Decision-making skills to independently exercise delegated authority to take action, and to recognize when it is appropriate to seek assistance from others when limits of delegated authority have been reachedPREFERRED-Ability to navigate third party reimbursement programs-PGY-2 Pharmacy Practice or Specialty Residency -Prior experience precepting and evaluating residents and studentsSPECIAL REQUIREMENT - THIS POSITION MAY BE SUBJECT TO A CRIMINAL BACKGROUND INVESTIGATION, DRUG SCREEN, LIVE SCAN FINGERPRINTING, MEDICAL EVALUATION CLEARANCE, AND FUNCTIONAL CAPACITY ASSESSMENT
