The Case Manager LCSW of the Case Management Support Program (CMSP) has extensive clinical and organizational knowledge as well as a working network of collaborative linkages with all levels of Substance Use Disorder and behavioral health treatment modalities and recovery-based support system. Services in Queens, NY will be provided for individuals with mental, emotional, and/or substance abuse problems, including abuse of alcohol, tobacco, and/or other drugs. Activities may include individual and group therapy, crisis intervention, case management, client advocacy, prevention, and education.
The Case Management Support Program (CMSP) is designed to provide services to Clients referred by the New York City Human Resources Administration (HRA) that have substance use disorders, are in transitional housing, have severe mental illness or other complicating behavior health issues. Services include CASAC assessment, treatment referrals, and intensive case management (75% field-based) to meet the complex and immediate needs of the Clients. Applying a Client-centered model, case managers would assist Clients toward achievement of: 1) Referral to substance use and/or mental health treatment; 2) Retention in substance use and/or other mental health treatment programs; 3) Income security by maintaining Case Assistance case continuity, gaining and retaining employment, or obtaining Federal Disability benefits.
Conduct a thorough psychosocial assessment of individuals with Substance Use Disorder, medical/mental health conditions, and/or homelessness
Counsel Clients in individual or group sessions to assist them in dealing with substance abuse, mental or physical illness, poverty, unemployment, or physical/emotional abuse
Monitor, evaluate, and record Client progress with respect to treatment goals
Advise Clients of their rights and responsibilities regarding Cash Assistance and other benefits
Assist Clients in coordinating a manageable treatment and work schedule
Help Clients to collect documentation for SSI and Housing applications as well as provide advocacy services throughout the application process
Provide community-based services in a variety of settings in a flexible manner, which may include Clients’ homes, at treatment facilities, in transitional housing sites, or in shelters; certain services must be offered beyond standard business hours to promote stabilization, income security, and engagement in programs
Provide intake and orientation services to assist Clients with understanding and exploring needs, strengths, interests, aptitudes, experience, capabilities, career goals, and family situations
Create, update, and maintain Clients’ Comprehensive Service Plans (CSPs) applying a Client-centered model specific to the unique needs of each individual with untreated or unstable substance use, mental health and/or medical conditions
Facilitate appropriate referrals to:
WeCARE for comprehensive assessment – also assist Clients to maintain engagement with WeCARE for HRA Clients with medical and/or mental health conditions
HRA or other appropriate employment, education/training or other services for HRA Clients who are fully employable and for whom treatment is not required
HRA’s Home Care Program, housing or other such services, if determined to be applicable
Provide referrals for a work or other applicable assignment concurrent with treatment if the Substance Use Disorder requires non-intensive treatment
Maintain ongoing face-to-face, telephone, and other collateral contact to support, encourage, and promote the Client’s commencement and ongoing compliance with the requirements of treatment and/or services outlined in the CSP, as well as to assist with mitigating barriers to engagement in services, wellness and recovery, as well as employability
Identify early warning signs of crisis and/or relapse and conduct timely crisis/relapse interventions
Work closely with Retention to monitor newly hired Clients for 6 months
Provide outreach to Clients who have difficulty with engagement to encourage enrollment and successful participation in program services and help address underlying issues to re-engage and make up missed program hours
Stress urgency and ownership, guiding Clients through a learn by doing process during their service planning and delivery
Provide vocational counseling, intervention, and program action planning and delivery, and advocate for Clients at treatment programs and work sites
Refer Client and/or family to community resources for housing or treatment to assist in recovery from mental or physical illness, following through to ensure service efficacy
Counsel or aid family members to assist them in understanding and supporting the Client
Plan or conduct programs to prevent substance abuse, combat social problems, or improve health or counseling services in the community
Observe HIPPA compliance in all instances including: while directly servicing a Client and/or referring them out to a community partner
Serve as a mental health subject matter expert, and serve as a consultant to CMSP Case Managers
Document activities, participation, compliance, and employment progress in the Electronic Case Management (ECM) systems
Knowledge, Skills and Abilities:
Knowledge of established evidence-based practices, including, but not limited to: Wellness Self-Management (WSM) and WSM Plus; Motivational Interviewing (MI); Cognitive Behavioral Therapy (CBT) techniques; trauma-informed care; and/or Individual Placement and Support (IPS)
Highly skilled in conducting psychosocial assessments, individual and group therapy with individuals with medical/mental health conditions, Substance Use Disorder, and/or homelessness
Knowledge of substance use treatment including Methadone detoxification and withdrawal symptoms and the use of Naloxone
Excellent assessment, treatment planning, and problem-resolution skills are necessary
Extensive clinical and organizational knowledge as well as a working network of collaborative linkages with all levels of Substance Use Disorder and behavioral health treatment modalities and recovery-based support systems
Highly organized and detail-oriented
Excellent communication, analytical, and interpersonal skills
Strong writing skills; competencies in writing goal-based/person-centered treatment plans and progress notes as evidenced by successful completion of writing sample
Must be computer literate with a working knowledge of MS Office products; knowledge of NYCWAY and STARS (Substance Abuse Tracking and Reporting System) is a plus
Ability to participate in CPR/First Aid training
Bilingual is a plus
Education: Graduate Degree in Social Work from an accredited institution, current NYS LCSW license
Experience: Minimum of three (3) years previous clinical experience or demonstrated clinical skills with assessment and treatment of addictions
Additional Salary Information: One position will be a lead for the case management team and also maintain a caseload.
